Origami Translation

I recently bought a wak with origami, but they did not send me the English translation. Can someone help me with the translation?

Takada Masayuki

Hi,
I can read most of it.
The parts that are written 'darker/bolder' are like a pre-printed form, and are the same on all examples of this type of origami.
It is a "kicho ninteisho" from NBTHK, dated November 11th 1967.
The description of the blade says "wakizashi, (something)-den (i.e., "made in the style of (something)), Takada Masayuki". See MAS1760 for reference.
The length is 1 shaku 2 sun 8 bun.
The left-most column is "(somebody)-sama", the owner's name.

Update:
The 2 kanji before "DEN" are MUMEI (unsigned).
So, the first line is "WAKIZASHI, MUMEI DEN (TAKADA MASAYUKI)".
